Do you have any period characters in the filename? Photoshop may think you've already given it an extension.
What happens if when you're doing File - Save As, you put the extension it should have on the end of the filename?
I don't know about Mac but do you have Windows configured to show you filename extensions? You should; that reduces confusion:
