Adding a job application in muse

Any suggestion in how to add a job application in muse? Our job application is very lengthy with check mark boxes etc, even if I could make it where you could click next and go to the next page would be fine and at the end have a submit button.

I guess I could use jotform but I don't want HR to get several email for the one application.
